Our Role as Geriatric Social Workers

Comprehensive Assessments

Biopsychosocial assessments: We conduct in depth-assessments with patients to support home health and hospice agency partners in maintaining our patients’ quality of life at the comfort of their home.

Information and Resource Guidance

Government Programs: We assist our patients in navigating local and state programs, helping them apply for benefits such as Social Security, Meals on Wheels, and healthcare services.

Assistive Technologies: We recommend in-home technologies and devices to improve seniors’ health and well-being.

Counseling Services

Emotional Support: we offer counseling to help seniors cope with issues such as legal documentation and the emotional stress of transitioning from independence to needing assistance.

Family Guidance: We provide advice to families on how to support their aging loved ones effectively.

Legal Advocacy
